Types of wheelchair

• Standard folding wheelchair — the everyday option for indoor and short outdoor use, self-propelled by the patient or pushed by someone else.
• Transport wheelchair — lighter, with smaller wheels, designed to be pushed rather than self-propelled. Easier to lift into a car boot for hospital visits.
• Commode wheelchair — with a removable seat and pan, for a patient who cannot walk to the bathroom. Often the single change that makes home care manageable.
• Reclining wheelchair — the backrest reclines and legs elevate, for patients who cannot sit fully upright for long or who need pressure relief.

Getting the size right

A wheelchair that is too wide lets the patient slide and lean; one that is too narrow is painful within an hour. Seat width should suit the patient's hips with a small amount of room either side, and seat depth should stop short of the back of the knee.

Using it safely at home

• Check the brakes every time before a transfer — most falls happen during transfers, not while sitting.
• Remove or swing away the footrests before the patient stands.
• Watch for pressure on the tailbone and heels if the patient sits for several hours; a cushion and regular repositioning matter.
• Thresholds and rugs are the usual obstacles in Indian homes — a small ramp is often worth arranging.

If transfers are becoming difficult or unsafe, a trained attendant for part of the day is often a better answer than a different chair.
